He has been charged with a string of driving offences, including failing to stop and assist after the crash, dangerous driving occasioning death, negligent driving occasioning death, and using a mobile phone when not permitted.
The arrest brings an end to a week-long search to find the alleged driver of a white Subaru outback.
Kokotatsios' partner told 9News she was stunned by his arrest.
9News understands East, 28, was returning from a night out and was just metres away from the Tamarama home he shared with his partner when he was struck at about 4am on Sunday.
East was born in New Zealand, studied at Harvard University in the US and had recently moved to Australia.
He worked for law firm Arnold Bloch Leibler in Sydney's CBD, who earlier this week described him as a "highly talented lawyer and popular and valued member of the firm, whose loss will be deeply felt" in an email to staff.
"The partners expressed heartfelt condolences to Mitch's parents, his partner and many close friends, both within and outside of Arnold Bloch Leibler," the email read.
East had an extensive career in law and was offered full scholarships for three of the world's most respected universities: Cambridge, New York and Harvard.
He made his name working on the Royal Commission of Inquiry into the Christchurch terrorist attack in 2020.
Kokotatsios was refused police bail and will appear at Parramatta Bail Court tomorrow.
