Shortly before 7.30pm yesterday, a blue Mazda CX-5 crashed into a tree and burst into flames at the intersection of Pendock Road and Sherringham Road in Cranebrook.
Bystanders rescued three male occupants from the burning car, which was extinguished by Fire and Rescue NSW.
"When police and ambulance attended there was a 12-year-old boy being assisted by some witnesses," Acting Inspector Adam Wilson from NSW Police said.
"It was later determined he suffered a broken leg and was a passenger of the car."
Before emergency services arrived, one of the car's occupants left the scene and was last seen on foot along Pendock Road.
"It was later determined that an 18-year-old man was placed into a shopping trolley and taken from the scene in that trolley to a house in Rosannah Way," Wilson said.
Paramedics were called a short time later to a home in Cranebrook, where a man could be heard yelling in distress, police say.
Paramedics were refused entry and contacted police, who were also refused entry.
"Police and ambulance could hear a man clearly crying out with distress and pain from inside the house and they forced entry for his welfare," Wilson said.
Two men inside, aged 40 and 55, allegedly hindered police, who used capsicum spray.
"During the course of the entry, the dog bit one police officer and was Tasered," Wilson said.
The dog was uninjured and left in a neighbour's care, police say.
Police found the vehicle's third occupant, an 18-year-old man, inside the home.
He was treated for a broken leg and head lacerations and taken to Westmead Hospital.
"They were extremely lucky, it could have been a lot worse and far more tragic," Wilson said.
"Particularly considering you have a 12-year-old boy who was a passenger at the time and was simply left at the scene at the time by his friends.
"These people who are involved in these offences have a number of friends and family who are very quick to assist them … and encourage them to commit these offences.
"As a result of that, they will continue to offend and we are very lucky this wasn't more serious and more tragic and it can very quickly develop into a fatal collision.
"I am calling on you (the friends and family) to start considering that course of action and steer them to some sort of constructive behaviour."
The 40-year-old and 55-year-old men were charged with hindering police.
They were bailed to appear in Penrith Local Court later this month.
Police say the car involved in the collision was stolen from a Cranebrook home during a break-in at about 3.45am on Wednesday.
The residents were asleep at the time and not disturbed.
The boy and the 18-year-old man were taken to hospital.
Both are in a serious but stable condition.
